Waverly High School Sports Physicals
Who: Students going into 8th grade or older, wishing to participate in a school sport
When: Saturday, August 7th from 8 am - Noon
Where: Waverly Urgent Care, Waverly, NE
To Schedule Appointment: Please call Waverly High School at 786-2765 before August 2, 2010. After August 2nd, you can contact the clinic at 786-5563 to schedule an appointment.
Cost: $45, please make check payable to Waverly Urgent Care.
All sports physicals for the 2010-2011 school year must be preformed after May 1, 2010 and before the student may practice.
Note: On the NSAA Student and Parent Consent Form do NOT check the boxes unless the student can NOT play those sports.
